PUBLIC NOTICE: AVAILABILITY OF IRS FORM 1095-B

In accordance with Internal Revenue Service (IRS) regulations, Networkers Funding, LLC is notifying all eligible current and former employees of their right to receive Form 1095-B, Health Coverage.

 

What is this form? Form 1095-B contains important information about your enrollment in health coverage during the calendar year. While the information on this form is reported to the IRS, you do not need to wait for this form to file your income tax return.

 

How to Request Your Copy: We no longer automatically mail paper copies of Form 1095-B. You may request a printed copy free of charge using any of the following methods:

Timeline: Once a request is received, a paper copy will be furnished to you within 30 days of the date the request is received.
